Registration for the 2016-2017 School Year
Montclair Elementary School Registration
Enrollment with OUSD and Registration at the school are two different things. Even if your child is enrolled, you must attend the on-site registration on August 19th, 2016. The on-site registration ensures that the teachers and staff have all of the relevant information they need to ensure the safety and welfare of our children on campus from the time the first bell rings.

New Families to Montclair Elementary School: Most of you have already enrolled with OUSD and registered with MES but we also encourage you to attend the New Parent Mixer, Thursday, August 18, 6:30-8:30 pm. Please join the sub-group “2016 Incoming Families,” on Member Hub to see additional details about the event. This is your opportunity to meet other new parents, meet our new school Principal Elizabeth Austin and new Assistant Principal Cheryl Ousley. If you are a new Kinder family, you will also get your classroom assignment. You have not “registered” with the school if you only enrolled with the district during the summer (and have been assigned to Montclair), or are still waiting for a school assignment. There are still forms for you to complete. These include our PTA Membership form, Fundraising forms, Volunteer forms and more. Be sure to check the list of forms below and download what you need.
Returning families: Registration for the 2016-2017 school year:
Parents need to download and complete the following forms: Acceptable Use of Technology Consent Forms, Parent Request for Language Assistance (if needed), Policies and Signatures pages (pages 7 &8) and Student Emergency Card. These documents can be found here: Fillable pdf registration forms You may also access the forms in member hub under “files”. New and returning families need to bring their child’s immunization record.
The Parent Guides and Health Guides will be distributed at Registration.
Please print and bring the following on Registration day (August 19): Technology form, the OUSD policies and signature page (pages 7 and 8), immunization record, and an emergency card.

Returning Families Registration Day: Friday, August 19 3 – 7pm, Multipurpose Room
You do not need to be present the entire time. You can come between 3 – 7pm, and turn in your registration documents listed above, get your teacher assignment, and sign up for school committees or volunteer teams such as the PTA, Dads’ club, and many others.
If you are not able to attend the registration on Friday, August 19, please send your registration documents with a friend, or bring your forms to the school office the week of August 15. Your class assignment will be given to you on Monday morning in the office. Your class assignment will NOT be emailed to you over the weekend.

PLEASE NOTE THE FORMS BELOW are updated for the 2016-2017 school year.
There is an online application through the district for Free and Reduced Price Meals for families that qualify, but we *all* need to fill it out in order for Montclair to receive the free breakfast and snacks that will be provided with the new cooking kitchen. Hard-copies are available in the office and will be added here as soon as we can get them scanned and loaded.
